Here are the forms you might need to complete your “Safer Environments” documentation. You will need to print them out and fill them out the “old fashioned way” with paper and pen. Then give them to Fr. Bo at Church by sliding them under his office door (the door that says “Pastorial Office” (yes, Pastoral is spelled wrong.)).
◀︎ This is the Basic Form that lets us know who you are. Everyone needs to fill one out.
◀︎ If you are currently under 18 years of age, fill in this one instead of the one for adults.
◀︎ This is a document showing that you as a volunteer represent the parish, and therefore you should act in a way that is befitting the Church. In order to sign this document you will need to read and agree with both the “Code of Pastoral Conduct” and the “Electronic Communication and Technology Policy”
◀︎ For those in “High Risk” Ministries we would like you to give us some references. These can be anyone who is not related to you. Yes, it can be other people from our parish.
◀︎ This is something you will need only if you are categorized as holding a “High Risk” position. A “Police Record Check” and a “Vulnerable Sector Search” will tell us if you have been arrested in the past and for what. If, for example, you were arrested we probably would still allow you to volunteer as a Perogy Pincher. However if you have been arrested for abusing children we would not accept you as a Sunday School Teacher. By clicking the button you will only get information on how to get your “Police Information Check & Vulnerable Sector Search” from your local Police Department.
◀︎ Annually you will be asked to make this attestation that you have not been arrested since your last Police Information Check, or you will declare that you have indeed been arrested since your last Police Information Check.
◀︎ This is required for “High Risk” Ministries, but recommended anyways for those of “General Risk”. An email will be sent to you from the Eparchial Safer Environments Coordinator which will give you access to the Online Training Course. This will take approximately 45 minutes time to complete.
